Try out our new system for electronically submitting your student’s athletic packet forms. Our new system provided by SportsNet Inc. allows you to easily fill out the athletic packet forms online, upload documents and automatically submit your information to your school.

Just click on your school link below, easily and quickly create a SportsNet account, and then within minutes, you will have completed this important step in ensuring your student can participate in extracurricular sporting activities.

If you have any questions on the new electronic forms, please don’t hesitate to call your school’s Health Aide. Important note: You will need to fill out the form in one visit so please allow about 10 minutes to complete the online process.

And of course, you can still submit hard copies by printing up the PDF forms to the right, filling them out, and then dropping off or mailing to your school.
